Lower back pain and getting back to the activities you enjoy
Lower back pain can leave you second-guessing everyday movements. You might hesitate before bending to put on your shoes, lifting a shopping bag or getting back into the gym.
If you are looking for back pain rehabilitation in Standish or Wigan, a useful starting point is a clear understanding of your symptoms and a plan that relates to your daily life.
What causes lower back pain
Back pain has many possible causes, and often there is no single obvious explanation. Many episodes improve within a few weeks, although pain can persist or return. An assessment is useful when symptoms affect daily life or are not improving. NHS back pain information.
Your appointment should give you room to describe the problem in your own words. Explain when it began, what has changed and what worries you. Mention any leg symptoms and relevant medical history rather than concentrating only on the painful spot. Our sciatica rehabilitation page explains how we approach radiating leg symptoms.
Should you keep moving
For most uncomplicated back pain, staying active and continuing manageable daily activities is encouraged. Prolonged bed rest is usually unhelpful. Choose movement you can tolerate, and get advice if exercises make symptoms worse. NHS advice on easing back pain.
“Stay active” can sound vague when you are sore. Ask your clinician to help turn it into a practical starting point: an activity, an amount and a way to review how you respond. That might centre on your commute, caring responsibilities or a leisure activity you want to resume.
What rehabilitation can include
NICE recommends advice tailored to the person and consideration of an exercise programme suited to their needs and capabilities. Manual therapy, such as massage or mobilisation, may be considered as part of a package that includes exercise. NICE guideline NG59.
The practical question is how a plan fits your week. Tell your clinician what equipment, space and time you actually have. If an exercise feels confusing or difficult to fit in, ask for clarification or an alternative. A programme should be something you understand well enough to use between appointments.
Do you need an MRI scan
A scan is not routinely recommended for uncomplicated lower back pain in a non-specialist setting. NICE advises considering imaging in specialist care when the result is likely to change management. New concerning symptoms or examination findings may change that decision. NICE advice on assessment and imaging.
Ask what a scan would help answer in your case. Equally, ask what the plan is if a scan is not needed, and when your symptoms should be reviewed.
Symptoms that need medical attention
Go to A&E immediately for new bladder or bowel changes, numbness around the genitals or anus, altered sexual sensation or function, or pain, tingling, weakness or numbness in both legs. Back pain with chest pain or following a serious accident also needs emergency assessment. Call 999 if necessary; do not drive yourself.
Get urgent GP or NHS 111 advice for fever, feeling unwell, or sudden severe or rapidly worsening pain. Unexplained weight loss or persistent night pain also needs medical review. NHS guidance on urgent back pain symptoms.
Common questions about lower back pain
Which exercise is best? The useful question is which exercise suits your current symptoms, ability and goals. Your plan may include different types of exercise rather than one universal movement.
Can sports massage be part of treatment? It may be considered alongside exercise and advice. Discuss what you want it to help with and how it fits the wider plan.
What should I track between appointments? Note activities you find difficult, any changes in symptoms and questions you want answered. Pick a meaningful goal, such as managing a work shift or returning to a class, so reviews stay connected to your life.
